Lazy Royal Gramma

I have a Royal Gramma presently in my quarantine tank that looks healthy but seldom swims. He mostly lies around on the bottom, often on his side or back. I have seen him swim normally on occasion but he has ben doing this for several months now. Is this expected behavior or should I attempt to treat this guy for a disease?

Are there any rocks or anything in this tank? If not he may not feel secure.

He was in my main tank (250gal) for about 6 months with plenty of hiding places but I was worried he may be sick so I moved him to a quarantine tank which has several artificial decorations in it. While he has not gotten any worse in the QT, I have not seen any improvement either.
